Shift: Full time, Days, Rotating weekend
Reporting to the Rehabilitation Supervisor, provides various
personal care services for Home Health patients; bathes and grooms
patient, maintains safety, assists with exercises and documents
Provides personal care for the patient, i.e., bathing, grooming,
skin care etc, in his/her home.
Performs simple technical procedures as outlined in the plan of
treatment and assignment instructions.
Assists with exercising and ambulating patient; utilizes correct
body mechanics while assisting with patient mobility; maintains
physical safety and comfort of patient.
Observes and reports significant findings regarding the
patient's condition to nurse or therapist; documents care given and
completes other appropriate records.
Assists with household services that are essential to the health
care of the patient, i.e., linen changes.
Reminds patients about medications that are ordinarily
self-administered; however, does not administer medications.
Participates in and supports Continuous Quality Improvement
Assumes responsibility for own professional growth by attending
continuing education classes as required.
Adheres to all Northwest Community Hospital standards, policies,
The level of knowledge normally obtained through the completion
of four years of high school, or the equivalent.
Valid certification from the Illinois Department of Public
Health; successful completion of competency validation exam.
Current C.P.R. certification from American Heath
A minimum of one year of home health experience is
Access to an automobile and valid Illinois driver's license.
Knowledge of and experience in the correct use of body
