Certified Nursing Assistant, PCT - Home Health

Job Description:

Shift: Full time, Days, Rotating weekend

Reporting to the Rehabilitation Supervisor, provides various personal care services for Home Health patients; bathes and grooms patient, maintains safety, assists with exercises and documents care.

  1. Provides personal care for the patient, i.e., bathing, grooming, skin care etc, in his/her home.

  2. Performs simple technical procedures as outlined in the plan of treatment and assignment instructions.

  3. Assists with exercising and ambulating patient; utilizes correct body mechanics while assisting with patient mobility; maintains physical safety and comfort of patient.

  4. Observes and reports significant findings regarding the patient's condition to nurse or therapist; documents care given and completes other appropriate records.

  5. Assists with household services that are essential to the health care of the patient, i.e., linen changes.

  6. Reminds patients about medications that are ordinarily self-administered; however, does not administer medications.

  7. Participates in and supports Continuous Quality Improvement activities.

  8. Assumes responsibility for own professional growth by attending continuing education classes as required.

  9. Adheres to all Northwest Community Hospital standards, policies, and procedures.

  10. The level of knowledge normally obtained through the completion of four years of high school, or the equivalent.

  11. Valid certification from the Illinois Department of Public Health; successful completion of competency validation exam.

  12. Current C.P.R. certification from American Heath Association.

  13. A minimum of one year of home health experience is preferred.

  14. Access to an automobile and valid Illinois driver's license.

  15. Knowledge of and experience in the correct use of body mechanics.

  16. The interpersonal skills necessary to interact with patients, families and hospital personnel.

  17. The communication skills necessary to speak and write in English.

  18. The ability to be empathetic to the patient's needs and well being.


  1. Moderate physical effort. Requires prolonged periods of standing, walking, bending or sitting. Requires frequent lifting of average weight material and lifting or transferring of patients, potentially without assistance.

  2. Flow of work and character of duties involves normal mental and visual attention much or all of the time.


  1. Conditions are acceptable. Several disagreeable elements or hazards are occasionally present; i.e., infectious diseases, high pressure medical gases, biohazardous materials and potentially adverse home environments. Patient homes may present physical barriers that impede access. There is frequent travel, potentially in inclement weather.
