Position Summary We are looking for a compassionate, community-oriented individual to join our team as a Community Health Navigator. In this role, you will help individuals and families in Cook County connect to essential healthcare services, including support with Medicaid enrollment and renewals. You donotneed prior experience with Medicaid redetermination we provide comprehensive training and ongoing support. What matters most is your ability to connect with people, communicate clearly, and care about improving access to healthcare in your community. This contractor role is ideal for someone who: Is bilingual in Spanish and English Enjoysworking directly with community members Isdependable andtimely Iscomfortableengagingwith people in public and community settings Is passionate about helping others access healthcare resources WhatYoullDo Support community members at local events, health fairs, and partner sites Provideone-on-one guidance in a welcoming and respectful manner Help individuals understand their healthcare coverage options Connect residents to Cook County Health services and resources Participate in training and team meetings to build your knowledge and skills WhatWereLooking For Fluency in Spanish and English (strongly preferred) Friendly, approachable, and culturally responsive communication skills Ability to work independently in community settings Commitment to health equity and serving diverse populations Experience in community outreach, customer service, healthcare, or social services is helpful but notrequired We Provide TrainingonMedicaid enrollment and redetermination Ongoing support from a collaborative team An opportunity to make a meaningful difference in your community If you are bilingual, passionate about helping others, and interested in growing your skills in community health,we encourage you to apply, even if youdontmeet every qualification listed.
