Job Description Hourly Pay Range: $38.67 - $59.94 - The hourly
pay rate offered is determined by a candidate's expertise and years
of experience, among other factors. Rehabilitation Clinical Liaison

Rehabilitation Clinical Liaison Location: Northwest Community
Hospital, Arlington Heights Full Time/Part Time: Full time Hours:
Monday-Friday, 8:30am - 5pm; Rotating weekends- 1 weekend per month
(May be additional coverage needs on a month with 5 weekends)
Required Travel: no Holidays: yes; 1 winter and 1 summer holiday
The Clinical Liaison- Rehabilitation serves as the representative
for the Acute Inpatient Rehabilitation Facilities (IRF's) within
the Endeavor System. They educate the community, build referral
networks, and cultivate physician and case manager relationships
both internally (within Endeavor Facilities) and externally. The
Clinical Liaison- Rehabilitation is responsible for completing
timely and accurate clinical assessment and admission of patients
including nursing, medical, therapy, and financial pre-admission
screening of patients referred to the Acute Inpatient
Rehabilitation programs. The Clinical Liaison assesses patients'
needs, reviews medical records, and communicates with care teams to
coordinate safe transfer of patients to the Inpatient
Rehabilitation Facilities within Endeavor. What you will do: Builds
strong relationships that help to grow business through an increase
of referrals and admissions. Markets IRF services both internally
and external to organization. Determines and builds new referral
opportunities. Serves as a personal contact to referring case
managers and physicians. Monitors and resolves customer service
issues between external customers and the hospital. Follows
patients during their stay to assure smooth course and
communication of care back to the referring physician. Works
closely with the IRF Directors to bring in external/internal
referrals to the Rehab Unit. Effectively utilizes all marketing
channels available to grow business. Achieves specified admissions
and revenue goals for IRF unit. Maintain patient referral data in
order to track success. Completes Preadmission screenings and other
required documentation in compliance with CMS and regulatory
requirements. Attends rounds and team meetings as necessary to
facilitate relationship with providers and referral sources. May
initiate insurance approval process for IRF Admission, as well as
update referral sources on status and coordinate peer to peer calls
as necessary. What you will need: Education: Bachelors or
associates degree in a Nursing, Physical, Occupational, or Speech
therapy related field Certification/License: Current Illinois
RN/LPN, OT/COTA, PT/PTA or similar preferred-license required.
Experience: 3 Years minimally working in a nursing or therapy
within an inpatient setting Unique or Preferred Skills: Background
in Nursing, Physical Therapy, Occupational Therapy or related
Healthcare field. Rehabilitation background recommended. Strong
customer service and marketing skills required. Possesses qualities
such as positive attitude, excellent interpersonal skills, good
communication skills (oral and written), highly organized,
excellent follow-through. Requires self-motivation, independent
learning, and resourcefulness to build and grow referral partners.
Ability to understand clinical-medical course, as well as navigate
